It begins with a one-on-one consultation with our nurse practitioner, in person or virtual, where we build a care plan around your goals. From there, choose our Wellness Membership, our preferred path for the most consistent, connected care, or à la carte visits based on what you need in the moment. However you begin, the goal is the same: helping you feel like yourself again.

Survivorship Support in Midlife
Our board-certified nurse practitioner is trained in both oncology and women's health, and she understands the unique experiences of survivors. She offers thoughtful, evidence-based care designed with your history in mind, including options that are safe for survivors and, when appropriate, carefully considered hormone therapy. She'll coordinate with your oncology team as needed, so your care stays connected and you can focus on feeling like yourself again.
Survivorship Consultation: a comprehensive initial visit, including review of your treatment history and records. $350
Survivorship Membership ($200/month): prescription refills, messaging, follow-up visits as needed, and coordination with your oncology team. Covers both hormonal and non-hormonal care.
À la carte follow-up visits: $275. (As an alternative to a membership)

Insurance & Medication Fulfillment
Do you accept insurance, or help with insurance coverage?
We do not accept insurance and are unable to facilitate coverage on your behalf. Any questions about insurance coverage for prescribed medications or bloodwork completed through Quest Laboratories should be directed to your individual insurance carrier.
How do I get updates on my prescription?
For any questions about filling or tracking a prescription, please reach out directly to the pharmacy it was sent to.
